Liability insurance, charging for needling

Hello all dry needlers! I currently work for a hospital system. I am considering dry needling some of my patients “on the side” “private pay” for maintenance as they continue their HEP once they are done seeing me for PT. I am looking for fellow clinicians who do this…..

What do you charge per session? How do you charge? Per body part or per session? 

Should I purchase liability insurance for myself to cover myself as I work “on the side”? I have looked into CM&F insurance. 

Do you have your patients or clients sign a dry needling consent form? Obviously I do for my part time hospital job.
