Carpal tunnel scarring and pillar pain

Does anyone have suggestions for where to start for chronic inflammation and thick scarring with pillar pain following carpal tunnel surgery? She had bilateral releases, has significant inflammation and scar tissue, burning quality of pain, MRI showed some fibrosis of palmaris longus tendon and neuritis of median nerve. Her surgery was 6 months ago.

Hi Krista.

I would start with some superficial needling around the palms and scars themselves, this will help reduce the hypersensitivity and begin to reduce some inflammation. If the patient can’t tolerate direct needling to this area start a little proximal and see if that helps, maybe at the wrist crease, AIN, palmaris longus and work your way into the palm. I would definitely add electrical stim as this will help increase the blood flow, wash out those sensitizing chemicals and you get a vibratory effect to help reduce scar adhesions. Follow dry needling treatment with nerve gliding and other therapeutic interventions.
